Draft Form 3005 revision
Prime Rate Laser Draft Form 3005
has been revised. A Policy Profiler settings change may be
required.
NOTE: The draft form number appears in small,
vertical print along the right edge of the draft form as: SI
FORM NO. 3005.
On the new revision of Prime Rate Laser Draft Form 3005, the
RENEWAL checkbox is no longer pre-printed on the form. Upon
reviewing your forms, IF you see that there is no
RENEWAL checkbox shown between the NEW POLICY and
ADDITIONAL PREMIUM checkboxes, then you will need to make
the following change in the program so that your laser
printer will print the RENEWAL checkbox when applicable:
- On the Policy Profiler
main menu, enter 61 (Agency Setup).
- Select Agency
Information.
- Press Page Down on your
keyboard once.
- On Page 2 of Agency
Information (top third of page) enter Y in the
Add Renewal Section to Draft 3005? field.
- Press F10 to save
the change. Then, at the confirmation message, select
Yes.
- Return to the Policy
Profiler main menu. You will need to exit Policy
Profiler to apply the change.
